Please complete this online form only after you have donated. Once we verify receipt of your donation, we'll forward the relevant information to Project HEALTH and/or PBHA, our two charitable beneficiaries. Shortly thereafter, Project HEALTH and/or PBHA will send you a tax letter. The donation amount that each organization will attribute to you in its tax letter is determined by the percentage breakdown that your runner selected. For example, if you donated $100 to your runner and your runner specified a 50/50 breakdown between PBHA and Project HEALTH, you'll receive one $50 tax letter from PBHA and one $50 tax letter from Project HEALTH. To our runners: Unfortunately, your Registration Fee cannot qualify for a tax letter because it is not a charitable gift.
